FIG. 5 is a bottom view thereof;

FIG. 6 is a left side view thereof with a model patient's head shown in broken lines; and,

FIG. 7 is a right side view thereof with the model patient's head.

Where included, the use of stippling and shading is provided to show contrast in materials and/or to differentiate between different areas of the design. The claimed design is not limited to the illustrative use of stippling and/or shading.

Where included, broken lines represent environmental structure and do not form a part of the claimed design. Where included, dotted lines are used to indicate the border between claimed and unclaimed subject matter.
