Description



FIG. 1 is a top plan view of the first embodiment of the document sheet with two horizontal perforations and two card shapes oriented as a bi-folding card showing my new design;

FIG. 2 is a top plan view of a second embodiment of the document sheet with two horizontal perforations and two card shapes oriented as a bi-folding card and the card shapes are covered with a laminate substrate patch thereof;

FIG. 3 is a top plan view of the third embodiment sheet with two horizontal perforations and four card shapes oriented as two bi-folding cards thereof;

FIG. 4 is a top plan view of the forth embodiment of the document sheet with two horizontal perforations and four card shapes oriented as two bi-folding cards and the card shapes are covered with a laminate substrate patch thereof; and,

FIG. 5 is a top plan view of the fifth document sheet with two horizontal perforations and four card shapes oriented as two bi-folding cards and the card shapes are covered with a laminate substrate patch thereof.

The perforations indicated by broken lines are part of the claimed design.
