FIG. 4 is a right side-elevational view thereof, the left side-elevational view being mirror symmetrical thereto;

FIG. 5 is a top view thereof; and,

FIG. 6 is a rear view thereof, not visible in use.

The ornamental design for a lever head and escutcheon for a wall-mount tub or shower faucet, as shown and described.
