Applicant has been affected by the COVID19 pandemic, which has caused unemployment issues and an inability to get funding at this point while they continue to develop their product. Because of this, we hereby request a waiver/refund of this fee for extension of time to file the statement of use, and a 2-month extension of the deadline to file any further statement of use or extension.


A fee payment in the amount of $125 will be submitted with the form, representing payment for 1 class.
