EXPLANATION OF FILING
Declaration As the trademark of the applicant was assigned, it was assigned from Wen Wei to FRICARE Medical (Dongguan) Co.,Ltd and CAREMATE MEDICAL DEVICE CO.,LTD After the assignment is successful, the assignee of registrant who needs the new certificate shall, after consulting the USPTO, respectfully request for the issuance of a new trademark certificate in the new owner by the trademark office It should be noted that Wen Wei assigned the trademark to two transferees ( FRICARE Medical (Dongguan) Co.,Ltd and CAREMATE MEDICAL DEVICE CO.,LTD). The applicant also filled in the two transferees when applying for assignment and submitted the relevant assignment agreement. However, when submitting this form, since the information has not been changed, it is found that only one holder information can be filled out when filling in the "Owner/Holder Information" step, and there is no blank to add other holders. In order to avoid any omission in the re- issued certificate, the statement is hereby made.
