Current identification: Medical and surgical instruments, namely, continuous auto-transfusion systems comprised of pumps, centrifuges, washing chambers, blood transfusion bags and tubing for surgical hemotherapy and intensive care

A fee payment in the amount of $125 will be submitted with the form, representing payment for 1 class.
