FORM INFORMATION

Applicant respectfully submits that due to a recent change of name the error in the United States Patent and Trademark Office database caused the wrong information to auto populate into the Request for an Extension of Time to File the Statement of Use form. 

Applicant believes that the United States Patent and Trademark Office had not yet updated their file for the mark "CSP" as the previous name of the Applicant was auto-populated into the Request to File an Extension of Time Form.  The Applicant is providing the recent Change of Name Recorded on Reel 6131 Frame 0489 that shows that a Change of Name was filed recently.  Applicant requests that the Extension of Time be Accepted or the Applicant be Allowed to Resubmit the Extension of Time.
