Hi Kim, I filled out the paperwork wrong when refilling for our trademark KEKA. (I did not realize I needed to renew after a few years, I am learning a valuable lesson).

 

Serial No. 85722715

 

I think I made the mistake of putting my name Mary Y Dale as the applicant and should have been the business name Keka Design, Inc. Please correct me if I am wrong is this what it means when asking for requirement for clarification of entity?

 

To fix my error, which form do I use on the response forms, I was not sure if I should use the first one or if I am suppose to submit the error by a letter.

 

I would appreciate any help or guidance.
