(2)  Enter the serial number of this application in the box labeled NUMBER.

(3)  Click on the SUBMIT button.

(4)  Find the document with a Mail/Create Date of 06/15/2011.

(5)  Click on the document “Offc Action Outgoing” next to that date.

(6) Click on the icon in the top right corner of the screen to download and print the document.
