As set forth in TMEP 1714 and 37 CFR 2.66 the applicants are requesting revival of this abandoned application and are including (1) The petition fee required by §2.6; (2) A statement, signed by someone with firsthand knowledge of the facts, that the delay in filing the statement of use on or before the due date was unintentional; (3) the required fees for the number of requests for extensions of time to file a statement of use that the applicant should have filed under §2.89 if the application had never been abandoned; (4) a statement of use under §2.88 This revival will NOT permit the filing of a statement of use more than 36 months after the date of issuance of the notice of allowance under section 13(b)(2) of the Act. The undersigned, is a person with firsthand knowledge of the facts, hereby asserts that the delay in filing the statement of use on or before the due date was unintentional


A fee payment in the amount of $100 will be submitted with the form, representing payment for the petition fee.

A fee payment in the amount of $150 will be submitted with the form, representing payment for the extension fee.

A fee payment in the amount of $100 will be submitted with the form, representing payment for the allegation of use for 1 class.

A fee payment in the total amount of $350 will be submitted.
