MISCELLANEOUS STATEMENTS
GenerationMed offers the services it claims to provide, for purposes of limited public market testing of its services. GenerationMed's services are provided to test, improve and refine operations, before final, full public release. GenerationMed provides three screenshot specimens as evidence of its bona fide use of its Mark in the ordinary course its trade. In consideration of the Trademark Law Revision Act of 1988 (TLRA), Public Law 100-667, 102 Stat. 3935: "The committee intends that the revised definition of 'use in commerce' be interpreted flexibly so as to encompass various genuine, but less traditional, trademark uses, such as those made in test markets..." [S. Rep. No. 515, 100th Cong. 2d Sess. 44-45 (1988)], GenerationMed claims bona fide use of its Mark for the services it claims to provide.


A fee payment in the amount of $100 will be submitted with the form, representing payment for the allegation of use for 1 class.


Declaration

Applicant requests registration of the above-identified trademark/service mark in the United States Patent and Trademark Office on the Principal Register established by the Act of July 5, 1946 (15 U.S.C. Section 1051 et seq., as amended). Applicant is the owner of the mark sought to be registered, and is using the mark in commerce on or in connection with the goods/services identified above, as evidenced by the attached specimen(s) showing the mark as used in commerce.

The undersigned, being hereby warned that willful false statements and the like so made are punishable by fine or imprisonment, or both, under 18 U.S.C.
