The following is submitted for registration number. 3742712 :

FORM INFORMATION

Please correct the ownership to CleanAlert, LLC, a limited liability corporation of Ohio.

There was a typographic error when submitting the Section 8 and 15 combined affidavit when it was filed in the name of CleanAlert, LLC,  a limited liability company of Ohio.

There was no change in ownership of the registration. Ownership is still retained by the entity in whose name the registration was issued.

Again, there is a continuity of chain of title. There was no change in ownership.
