The trademark specialist has determined an unexplained break in the chain of title that must be clarified.  Caldwell Consumer Health, LLC, a New Jersey LLC, is the true owner of the trademark registration No. 1609550 for the mark "CONCEPTROL."  The ownership chain of title for Caldwell Consumer Health, LLC has remained unchanged by Caldwell Consumer Health, LLC, a New Jersey LLC.  The Section 8 Affidavit filed by Caldwell Consumer Health, LLC, a New Jersey LLC, was erroneously filed as Caldwell Consumer Health, LLC, a North Carolina LLC.  The error, an unexplained break in the chain of title from a New Jersey LLC to a North Carolina LLC was due to a clerical error in populating the corporate citizenship question of the online Section 8 Affidavit form.

Please amend the record to reflect Caldwell Consumer Health, LLC, a New Jersey LLC, as the true owner of Trademark Registration No. 1609550 for the Section 8 Affidavit Application as filed on February 7, 2011.

The true owner, Caldwell Consumer Health, LLC, a New Jersey LLC, confirms that it was the true owner of Trademark Registration No. 1609550, for the mark "CONCEPTROL,"  on the filing of the the Section 8 Affidavit Application as filed on February 7, 2011 and that John P. Maldjian, Attorney of Record and NJ State Bar Member, was duly authorized to sign the Section 8 Affidavit Application on February 7, 2011.
